Specialist Support Coordination

I have a Bachelor of Social Science majoring in Behavioral Science level qualified to provide all levels of NDIS support coordination, if you have Psychosocial Recovery Coaching in your plan. You will receive both structured Coaching sessions AND Support Coordination.

TO NOTE*** My aim- to be responsive. I have heard the feedback from working in larger organizations, these are not just words to read on a website, I have intension and passion to support all the participants on my caseload. I deliberately keep my caseload small, so I am focused on you and your needs. You are at the Centre of the support. The participants I walk along side as a Coordinator describe me at as ” the first person who actually cared about their needs” Passionate and ready to support them no matter what” These again are not just words to advertise my work. I aim for face-to-face interactions and meet in home and build genuine connections.

There are different levels of support coordination that can be included in your plan:

  1. Support coordination – I will help you build your capacity in understanding how to use your plan. Further to this, help you to understand service agreements and maintain and build relationships with your providers of supports.
  2. Specialist support coordination –I will work with you and your family or providers at this level of support intensively. Additionally, I will work with you in a short term targeted intensive support and will perform complex planning with you.
